To meet and support members with ongoing, chronic or emergent conditions and the unique health needs they create, we offer two discrete programs with individual care teams: the Texas Health Aetna Care Team, and Aetna’s Multidisciplinary Care Teams (MDCT).
Texas Health Aetna Care Team: we take your health personally.
Meet Kristen and Kathy, the people who make it simpler to be a healthier you. As a registered nurse and a care management associate, respectively, they’re on call and on your side, making sure you get the right help, care and advice on all your health needs.*
They’re here to help you navigate care and services for new or existing conditions: from diabetes from diabetes to hypertension, kidney disease, joint and back pain. They’ll also address gaps in care and even health education, while helping you introduce (and hopefully) lifestyle, dietary and behavioral changes. Plus, they’ll assist in guiding you through the system, making specialist and diagnostic appointments, coordinating care and follow-up with your doctor, as well as ensuring you get the right prescriptions and refills.
So, when you get their call, or text, please respond. You’ll be glad you did.
Aetna MDCT: coordinated condition management with you at heart.
Composed of nurses, social workers, diabetes health educators and pharmacists, the professionals on our Multi-disciplinary Care Teams specialize and focus on members who live with intense, complex and chronic conditions and have a particularly high level of need, ensuring you get the care you need and deserve.
Features of the program include:
- An individual care manager to coordinate and communicate with your doctor, hospital and specialists, answer questions, monitor your condition, simplify how the system works for you.
- Help in finding in-network providers such as local doctors, specialists, mental health providers, clinics, and diagnostic testing and imaging locations.
- Help with appointments, prescriptions, scheduling hospital visits, follow-ups, routine screening reminders and referrals, therapy – even claims.
- Regular check-ins with your care manager by phone, virtual chat and home visits.
- Connecting you with convenient care options such as Anytime-MD, House Calls via DispatchHealth, CVS MinuteClinic® and HealthHubTM locations plus referrals to community resources.
- Education on chronic disease and condition management and regular assistance via automated text-message reminders.
